ENABLE
This service aims to provide support and rehabilitation to people with progressive neurological conditions who might have complex physical or cognitive disabilities. The ENABLE team helps people who are newly diagnosed as well as those who may have had their condition for some time and who are trying to maintain their independent lifestyles. The team also provides support, advice and information to family and carers. ENABLE receives referrals from Consultants, GPs and other health professionals. For the first contact with the ENABLE Team, a referral from a professional is usually required.

Homeward
Home ward Ealing helps people during a period of severe or sudden illness, or when they have been discharged from a general hospital, so they can recover and remain well at home. The service is for people aged 18 or over who live in the London Borough of Ealing or who are registered with an Ealing GP (family doctor). Similar services are sometimes referred to as ‘intermediate care’, ‘integrated care’ or ‘virtual wards’. If you think you would benefit from our service, you should discuss this with your GP.

Musculoskeletal (MSk) Service
The Musculoskeletal (MSk) and physiotherapy service provides assessment and treatment for adults with problems with their muscles, ligaments, tendons, joints, discs or nerves.
We aim to improve your strength, mobility and independence, giving you the power to understand and better manage your condition. As well as treating a specific injury or illness, we will also assess and identify ways to help improve your general well being and quality of life.
